Athletics Eligibility Forms

All student-athletes are required to complete new Pre-Participation Physical Forms each year before participating in any MSHSAA activities, practices, or competitions. These forms help ensure the health and safety of every participant and must be submitted prior to the start of the season

All students participating in MSHSAA activities must complete required pre-participation documentation each year before taking part in practices or competitions. This includes proof of insurance, consent, and medical history forms. Please note: the Pre-Participation Physical Examination Form is valid for two years from the date of the physical. Be sure all required forms are completed and submitted by the designated deadline to ensure eligibility for participation.
